Transfer Call not working

We have an assistant that transfer call feature and many times around 25% of the time when the assistant tries to transfer a call, the call dropped and the status on the log shows silence timeout. And we don't know how to fix it. Can you please help me urgently on this issue because this is causing real problem for our client. Basically, assistant says on the call, I am transferring your call, but then nothing happens. And customer thinks it's being transferred, but after sometimes the call ends with silence timeout status. We have tried to increase the silence timeout to 2 minutes as well, but that did not help. Because ideally it should transfer the call when it says it's transferring the call. And I don't see the tools being triggered. And it does not happen all the time, but it happens enough times to cause a problem for our client. Any help on this issue would be highly appreciated.
