AI loops and does not transfer call

@Vapi hi team, staff member calls in sick which AI is instructed to transfer to manager. The AI repeatedly offered to connect the user to a duty manager for immediate action, despite the user's repeated affirmations. The enquiry was not resolved as the AI continued to loop, and the call ultimately ended with the customer hanging up.

Could we please take a look into why this is happening?

00:12 - AI mentioned it will connect them to manager. Caller confirms "yes" to be transferred. AI then says "Oh, so sorry. Could you please clarify your query? Or let me know how I can assist you today?" and the conversation loops another 6 times. The transfer doesnt go through.
