Transfer Call ends the call instead of transferring it

I'm setting up call forwarding for the case when a user asks to talk to a human. In the log I can see that the call is being transferred and the end of call reason is "assistant-forwarded-call". However, the conversation ends immediately after that.

Do I miss something in the setup process?

I have Forwarding Phone Number set up. And in the prompt I instruct the model to use transfer call function to my number.
