Facing issues with Outbound Calls in Make

I am fairly new to Make. I retrieved and forwarded data such as PhoneID, CustomerID, etc. But VAPI was not able to make the outbound call. I'm sure it's a simple fix but I wasn't able to find it on my own. I would highly appreciate it if someone could help! Thanks!
